Diabetes

This measures group is to be reported for patients aged 18 through 75 years with diabetes mellitus receiving office or other outpatient services, nursing facility care, domiciliary/rest home/custodial care services, or medical nutrition therapy.

PQRS Group Measures:

#01. Hemoglobin A1c poor control in diabetes mellitus

#02. Low density lipoprotein cholesterol control in diabetes mellitus

#03. High blood pressure control in diabetes mellitus

#117. Dilated eye exam in diabetic patient

#119. Urine screening for microalbumin or medical attention for nephropathy in diabetic patients

#163. Foot exam

Diabetes Mellitus Example:

Primary care office visit for a new patient with newly diagnosed diabetes mellitus: A1c = lab drawn, result unknown, prior result not available; LDL-C=110; today’s BP = 140/80; referred to eye care professional (optometrist or ophthalmologist) for dilated eye exam; urine protein screening performed = negative; foot exam performed.
