Preventive Care
This measures group is to be reported for patients aged 50 years and older receiving office or other outpatient services (CPT codes: 99201, 99202, 99203, 99204, 99205, 99212, 99213, 99214, 99215). Some measures in the Preventive Care Measures Group apply only to a subset of these patients (eg, Urinary Incontinence Assessment applies only to female patients aged 65 years and older.)).
PQRS Group Measures:
#39. Screening or therapy for osteoporosis for women aged 65 years and older
#48. Assessment of presence or absence of urinary incontinence in women aged 65 years and older
#110. Influenza immunization for patients 50 years and older
#111. Pneumonia vaccination for patients 65 years and older
#112. Screening mammography for women aged 50 through 69 years
#113. Colorectal cancer screening for patients aged 50 through 75 years
#128. Body mass index (BMI) screening and follow-up
#173. Unhealthy alcohol use screening
#226. Tobacco Use: Screening and Cessation Intervention

Preventive Care Example:
Primary care office visit for a 67 year old female, established patient. Record indicates patient had a DXA done at age 62, with results documented as within normal limits; denies urinary incontinence; record indicates influenza vaccination at a previous visit in January of this year; pneumonia vaccination administered last year; results of last month’s mammogram and last week’s FOBT reviewed with patient; denies tobacco use; today’s BMI measurement = 24
