PQRS Feedback Reports

You have three choices to obtain the 2010 PQRS and eRx feedback reports from Medicare:

• Call your Medicare carrier and ask them to email you these reports. Your Medicare carrier will ask you for your individual NPI, your group TIN and your Provider Transaction Access Number (PTAN).
• Request individual provider reports at the CMS Communication Support Page
• Request group provider reports at the Physician and Other Health Care Professionals Quality Reporting Portal. If you do not have an account, you will need to register your group with IACS.
