Chronic Kidney Disease

This measures group is to be reported for patients aged 18 years and older with advanced CKD (stage 4 or 5, not receiving Renal Replacement Therapy [RRT]) receiving office or other outpatient services.

PQRS Group Measures:

#121. Laboratory testing (calcium, phosphorus, intact parathyroid hormone(iPTH) and lipid profile)

#122. Blood pressure management

#123. Plan of care - elevated hemoglobin for patients receiving erythropoiesis-stimulating agents (ESA)

#153. Referral for arteriovenous (AV) fistula

CKD Example:

Stage 5 CKD patient, not receiving RRT, office visit: lab tests ordered on last visit and results documented in the chart; known hypertensive with documented plan of care for hypertension; Hgb = 14 and patient is receiving ESA and has a plan of care documented for elevated hemoglobin level;  referred for AV fistula.
