Back Pain Example:
Initial office visit for a new patient with newly diagnosed back pain: back pain and function assessed including pain assessment, functional status, patient history with notation of no “red flags”, prior treatment and response assessment and employment status; physical exam performed ; patient counseled to resume normal activities; patient advised against bed rest lasting four days or longer.
